About PHC

Care that meets people where they are.

For 20 years, PHC has been a trusted community anchor for people underserved by traditional healthcare systems.
Our Mission

Our mission is to provide accessible, affordable healthcare for people who are underserved, at-risk, or experiencing homelessness — and to meet them where they are.

Community Need

The gap in quality healthcare.

Traditional care isn’t built for the realities of poverty and homelessness. People navigating unstable housing need integrated care that treats the entire individual — physical, mental, and emotional health together. PHC’s model meets that need: client-centered care paired with help enrolling in government aid and community support, so people can thrive.

Our Story

Built over 20 years.

Peninsula Healthcare Connection began in 2006 as a collaboration between the Community Working Group, Palo Alto Medical Foundation, and the Stanford University School of Medicine. From a single clinic at the Opportunity Center in Palo Alto, we set out to make quality care reachable for people the traditional system overlooks.

Twenty years later, that mission still drives us. Through lasting partnerships with Stanford Health Care and Palo Alto Medical Foundation, volunteer physicians bring their expertise to our patients in the clinic and on the street.

Give Through Your Assets

Some of the most meaningful gifts come through the assets you’ve built over a lifetime. A gift of stock, retirement assets, or a legacy commitment can help ensure that individuals and families across our community continue to have access to healthcare, food, housing support, and the resources they need to thrive.

Whether you’re ready to make a gift or simply exploring your options, we’d be honored to start the conversation.
