Ability Link helps older adults and people with disabilities age in place with confident, personalized care coordination.

Overview

Ability Link, a program of Peninsula Healthcare Connection (PHC), provides personalized care coordination for older adults, individuals with disabilities, and families navigating complex care needs in Santa Clara County.

Through licensed Life Care Managers, Ability Link supports aging-in-place by helping individuals manage healthcare, plan for life transitions, and access the right services with confidence. Services are available through private pay, employee assistance programs, and select insurance partners.
