Our Services

Primary Care

Family physicians provide comprehensive day-to-day healthcare services. Referrals to specialized healthcare and diagnostic testing are also available.

Behavioral Health

A Board Certified Psychiatrist and licensed counselors facilitate an array of behavioral health services including, medication management and therapy.

Community Outreach

Community Outreach Activities help distribute basic needs essentials to low-income and unhoused residents in need and facilitate wrap-around services.

COVID-19 Services

PHC offers free, COVID-19 testing and vaccinations. PHC distributes free COVID-19 self-tests for patients upon request and availability.

OUR PROGRAM

New Directions

The New Directions program nurtures personal stability and well-being in Santa Clara County’s most medically and psychosocially vulnerable populations. New Directions provides community-based behavioral health, case management, and other supportive services to clients referred from Santa Clara County partner agencies, managed Medi-Cal plans, community clinics, hospitals and local healthcare providers.
